Liability Release

I understand that there are risks of physical injury associated with and inherent to the activity of dance. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/or cause of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to *AIM, its officers, employees, instructors
and all affiliated entities. I hereby agree to release *AIM and hold *AIM harmless of all liability. This acknowledgement is voluntary and I assume full responsibility for all risk of physical injury arising out of active participation in dance.

Rules and Behavior Policies (please initial clearly)

  *AIM is committed to equality and does not discriminate against students based on race, color, national origin, religion, gender, age, sexual orientation, disability or veteran status. *AIM is committed to welcoming, recruiting and advancing students of all levels thus increasing the diversity and excellence of our dancers. All students are expected to treat each other and the instructors
with respect at all times. *AIM reserved the right to terminate attendance privileges in the event of improper behavior.

  The unwelcome verbal, visual or physical conduct of a sexual nature that is severe or pervasive affecting working conditions or creating a hostile work or social environment known as “Sexual Harassment” will not be tolerated at *AIM. ALL students are expected to treat each other and all instructors with respect at all times. *AIM reserves the right to terminate attendance privileges in the event of improper behavior.

  I agree to abide by the rules and policies of *AIM. This includes, but not limited to, behavior that exhibits respect to fellow students and the instructor(s) as well as staff and visitors.

  I agree that if I arrive late to class, it is my responsibility to contact my fellow students after class is completed to catch-up on what was missed. If arriving more than 25 minutes late, you are welcome to schedule a private lesson at a later date with your instructor for an additional fee.

Photo/Video Media Policies & Release (please initial clearly)
I understand that all classes/activities/events at *AIM may be recorded for digital and printed marketing materials. I acknowledge that I am giving unrestricted PERMISSION for my image to be used in print, video (including sound bites), and digital media at *AIM without further notification. This includes advertising and promotion through the use of Social Media.

I DO NOT wish for my photo/video to be taken and I understand that it is my responsibility to remove myself from the view of the camera when *AIM employees begin recording videos or taking photos.

I understand that visitors are permitted to sit and watch classes but are NOT permitted to record any class through the use of cell phones, cameras, video cameras or any other technology. Students ONLY are permitted to record at the end of the class AND upon the instructor’s directions ONLY the material taught within that class. The recorded material is to be used only for the
student’s personal use of practice and NOT to be uploaded and/or shared through email, personal SMS and/or Social Media.

Student Performance Participation
Are you enrolling in a Student Performance Class? If yes, please read and initial the following:

I have been given a Student Performance Information Sheet and agree to the rules and instructions pertaining to these specific classes. I have signed and have been given a copy of said information.

Financial Responsibilities and Policies
I understand and agree to the following:

• No credit will be given for classes and/or workshops that are missed by the student.
• Canceled classes will be rescheduled on a separate day by the instructor and communicated to all students with advanced notice.
• Weather related cancellations will be given with as much advance notice as possible and will be communicated via student email, our website (www.danceataim.com) and our social media pages (Facebook and Instagram)
• Any and all online deals are for NEW students only and are limited to one (1) per student. We cannot honor multiple vouchers for the same person or existing students. We only honor the amount paid for the deal and the student will be responsible for any difference in amount if applicable.
•  If monthly passes are purchased, the next month will be due the same date of the following month.
•  A late fee of $10 will be applied if tuition payment is received 10+ days after payment is due.
• Regarding Student Performance Classes, monthly performance class passes already purchased are non-refundable.
• Written notice is required to terminate enrollment before the following month's tuition is due.
• ALL payments are accepted via cash or credit/debit card. Our Auto Pay option MUST be requested by students and must complete an Auto Pay Authorization form.
